Payment for Cancelled Class A Shares – Payment Request Form
Notice to persons who were holders of Class A Preference Shares of The Ottawa Curling Club, Limited.
The Class A Preference Shares of The Ottawa Curling Club, Limited were cancelled by the Shareholders at the meeting of Shareholders held on October 18, 2025. All former Class A preference shareholders shall be eligible to receive payment of $0.625 per share. To receive such payment, said Class A preference shareholders must inform the Corporation in writing of their intent to receive payment no later than October 27, 2026.
A copy of this Payment Request Form for cancelled Class A Preference Shares was sent by Canada Post on December 8, 2025 to Class A Preference Shareholders of the Corporation for whom the corporation had an address recorded in the books of the corporation. If you were a holder of Class A Preference Shares and did not receive the Payment Request Form, please download the form here and send to the corporation as soon as possible.
The completed Payment Request may be returned to the Corporation by e-mail: This email address is being protected from spambots. You need JavaScript enabled to view it.; or by Canada Post: to 440 O’Connor Street, Ottawa, Ontario K2P 1W4, or by courier to 440 O’Connor Street Ottawa, Ontario K2P 1W4. The Payment Request is not complete until actually received by the Corporation.
Deadline: The Completed Payment Request Form must be received by the Corporation on or before October 27, 2026. No Payment Requests for cancelled Class A Preference Shares of the Corporation will be accepted after that date.
CLASS B SHAREHOLDERS
Payment for Cancelled Class B Shares – Payment Request Form
The Class B Preference Shares were cancelled by the Shareholders at the meeting of Shareholders held on October 18, 2025. All former Class B preference shareholders shall be eligible to receive payment of $0.01.
This Payment Request for cancelled Class B Shares was posted to the Ottawa Curling Club website https://ottawacurlingclub.ca/ on December 4, 2025, and by order of the Board of Directors, there will be no further or other Notice to Class B Shareholders.
Class B Shares can only be redeemed by attendance in person at the headquarters of the corporation located at 440 O'Connor Street in Ottawa Ontario and is by appointment only. Appointments may be made by calling the General Manager at 613.234.4119
Deadline: The Class B Payment Request must be received by the Corporation at 440 O'Connor Street in Ottawa Ontario on or before October 27, 2026. No Payment Requests for cancelled Class B shares will be accepted after that date.
